Wrestlers who have competed in a qualifier but will not be able to compete in the respective U16 / Junior State Championship and want to be considered for the Ohio U16 or Junior National Teams, must complete and submit this form with the $50 Petition Fee Per Style PRIOR TO THE START of their respective State Tournament. NO EXCEPTIONS! 

If due to a weight change for a Junior at the State Tournament, the form is due no later than the end of the respective State Tournament you are petitioning for with the $50 petition fee, to the Head Table.

Forms may be emailed to Adam Gilmore ([email protected]) or Chris Kallai ([email protected]) in advance but will not be processed unless the $50 Petition Fee is received prior to the start of their respective State Tournament. No forms or fees will be accepted if mailed.  

The $50.00 petition fee is NON-REFUNDABLE. (Cash or Check made payable to USA Wrestling-Ohio)

Wrestlers who want to petition for both styles must submit separate petition forms and petition fees for each style.

Requests for medical waivers must be accompanied by appropriate documentation
